British Study Reveals Alarming E‑Scooter Injury Patterns

Recent research conducted by a team of trauma surgeons in the United Kingdom compared severe injuries sustained by electric‑scooter users (often called “steppers”) with those suffered by motorcyclists and cyclists. By linking the national trauma register of England and Wales (covering patients admitted for at least three days, intensive‑care stays, or in‑hospital deaths) with self‑reported incidents from the rental company Voi, the investigators assembled a dataset that spans 2020‑2022 and includes 580 e‑scooter cases, 7,027 motorcyclist cases and 7,640 cyclist cases.

Head Trauma Dominates E‑Scooter Cases

More than one‑third of the e‑scooter victims recorded in the trauma register suffered head injuries. This proportion is roughly three times higher than the share observed among motorcyclists and almost double the share among cyclists. While bone fractures were more common in motorcyclists (about 54 % of their injuries), e‑scooter riders displayed a higher incidence of internal organ damage and vascular tears, underscoring a distinct injury profile.

Why Helmets Matter – and Why They’re Rarely Worn

Helmet usage emerged as a critical differentiator. Only 5.9 % of e‑scooter patients in the trauma register were wearing a helmet at the time of the crash, compared with 75.7 % of motorcyclists and 45 % of cyclists. Among Voi’s voluntarily reported incidents, the helmet rate dropped even further to just eight per 1,000 rides. The combination of a higher centre of gravity, small wheels without suspension and the tendency to be thrown over the handlebars during abrupt braking creates a scenario where the rider’s head is the first point of contact.

Vulnerable Groups: Children and Women

Children under 16 accounted for more than 16 % of the e‑scooter injuries, a share that exceeds the percentages seen in the motorcyclist (6.2 %) and cyclist (8.9 %) cohorts. Younger riders are more likely to sustain abdominal injuries because the handlebar aligns with their mid‑section. In the Voi data, women experienced severe injuries at roughly twice the rate of men, even though men performed the majority of rides. Possible explanations include differing route choices (e.g., sidewalks or unpaved surfaces) and the fact that most e‑scooter designs are based on an average male body.

Limitations and Practical Takeaways

The study does not provide a per‑kilometre or per‑ride risk estimate, and it lacks information on speed, alcohol consumption, road surface conditions and rider behaviour. Consequently, while the association between e‑scooter use and heightened brain injury risk is clear, causality cannot be definitively proven. Nonetheless, the findings reinforce the importance of mandatory helmet use, especially for younger riders, and suggest that policymakers should consider stricter safety standards for e‑scooters.
